1)This form is to be filled up by the players who are interested to play & shall be treated as an application for participation. The final confirmation of your participation is a sole decision of the Committee & the same will be conveyed to each player. If in case of withdrawal of the player due to any reason, the participation fees is non-refundable.
2)By filling up this form you accept to abide by the rules and regulations formed by the committee. Any decision made by the committee will be final & binding on each player.
3)Up-Front payment of Participation fees is compulsory (Rs. 1500/-) per player. No cash payments will be accepted. Your participation will be considered after making payment only.
4)No Registered State Or National Players Are Allowed.
5)In all cases decision of the committee will be final.
