1) This form is to be filled up by players interested to play and shall be treated as an application for participation. Final confirmation is the sole decision of the Committee. If participation is not confirmed by the Committee, the participation fee will be refunded fully. The participation fee is non-refundable in all other cases.
2) By filling up this form you accept the rules and regulations formed by the Committee. Any decision made by the Committee will be final and binding on each player.
3) Up-front payment of participation fees is compulsory (Rs. 2500/-). No cash payments will be accepted. Participation will be considered only after payment.
4) In all cases, the decision of the Committee will be final.
